Credentialing for physical therapists: a practical guide

Credentialing for physical therapists is the process of enrolling each PT with the insurance plans you want to bill, so you can see insured patients and be reimbursed. It covers your CAQH profile, Medicare enrollment through PECOS, and commercial and Medicaid panels — and, like all credentialing, it generally takes 60–120 days per payer.

What PT credentialing involves

  • CAQH — the universal profile most commercial payers pull from. It has to be complete and attested before applications move.
  • Medicare (PECOS) — physical therapists in private practice enroll through PECOS to bill Medicare Part B. This is often the longest single step, so start it early.
  • Commercial payers — each plan (Aetna, UnitedHealthcare, Cigna, the Blues, and others) has its own application and panel.
  • Medicaid — state-specific, with its own portal, rules, and timeline.
  • Group vs. individual — if you bill under a group NPI, the practice and each provider both need to be enrolled and linked.

How long PT credentialing takes

Expect 60–120 days per payer. Medicare and Medicaid timelines swing the most by state and backlog. The biggest avoidable delays come from an incomplete CAQH, a mismatched or missing NPI, and applications that aren’t followed up on.

Should you DIY or hire a service?

You can credential a PT practice yourself — it’s paperwork, not magic. The question is whether the hours are worth it. Each payer is a separate application, follow-up loop, and revalidation date to track. For a solo PT with one or two panels, DIY can work. For a multi-provider clinic, or anyone who’d rather treat patients than chase payer portals, a credentialing service is usually the better trade — you stay independently in-network without losing the time.

PT credentialing is the same core process as any provider’s — CAQH, Medicare, commercial, Medicaid — with a few PT-specific quirks around PECOS and group billing. Start early, keep CAQH clean, and track every application and revalidation date.
